Outcomes based commissioning
NHS Croydon Clinical Commissioning Group (CCG) and Croydon Council are working together to deliver a new approach to commissioning services for people over 65.
Known as Outcomes Based Commissioning (OBC), this is an exciting and innovative approach that promotes the integration of health and social care services in order to transform the way services are provided for older people in Croydon.
Our aim is to transform the way services are provided by putting what matters most to older people and their families at the heart of everything we do. We want to deliver services that meet the patients' needs with greater emphasis on prevention and by working together improving the quality of care provided to older people.
People have already told us that it is important to:
- Stay Healthy, active and independent for as long as possible
- Get more care and support tailored to their individual needs
- Get access to the best quality care so they can live how they choose
- Have support from professionals with specialist knowledge to understand how health and social care affects individuals
- Be supported to manage long term conditions

We held a public event on Wednesday 24 June 2015 at 6pm in Fairfield Halls, about 30 members of the public and community groups attended. You can read the presentation below, we've also included a link to a King's fund video about integration that we showed and a large print version of the draft outcomes framework.

Join a focus group and give us your views
We are asking for local people, patients and their carers or families to give us their views about services at a series of focus groups targeting three specific areas:
- long term conditions
- carers of older people
- the next generation: people aged 50 to 64
